About CHRC

Community Health Resource Center is a family-centered, nonprofit organization, established to address disparities in low-income and rural communities, of basic health care needs and resources available and not available, within the communities. We believe that communities could best benefit by having a single point of access to services, coordination of services and resources for children and their families, Community Health Resource Center was organized in 1999 to meet their needs.

It is the vision of Community Health Resource Center to improve the health, education and socio-economic status of people living in low income and underserved areas. Believing that, providing health education and case management services will reduce fragmented care and improve healthcare status; providing tutoring to the youth and children will enhance academic performance; and life skills and skills training will promote self-sufficiency, growth of individuals and economic development. It is our goal to bridge the gaps, providing the family and the communities a comprehensive approach to improving their health, binding the families and empowering them to access resources available within the community and surrounding areas.


Programs and Services:


1. Case Management - To identify needs and develop an individual plan of care/services.
a. Linking the child/individuals to primary care physicians or medical home
b. Be a liaison between the indivdual/family and the providers of services.
c. Supporting the families assuming the primary responsibilty for their case management.
d. Identify members of a medical home team within the communities.




2. Provide Health Education within the community:
a. Diabetes
b. High Blood Pressure
c. Obesity
d. Healthy Diets and Physical Activities
e. Heart Disease





3. Provide Self- Sufficient Assistance
a. Life Skills
b. Parenting Classes
c. Financial Literacy and more
d. College Preparatory Assistance
